Story so far
The medicated lotion is the sole reward from Carl's Hitchhiker achievement, received during his box-opening session in The Ceasefire. It is explicitly unenchanted — an ordinary bottle of lotion — meant for him to "take care of" the wound left by his new tattoo. The achievement provides no information about what the tattoo actually is, which is precisely what makes both it and the mundane reward feel ominous to Carl.
